Ozone in Dentistry

Ozone consists of three oxygen atoms.  The elemental form of oxygen occurs naturally due to ultraviolet light in combination with atmospheric electrical discharge or lightning, which causes a temporary recombination of oxygen atoms into groups of three. An ozone generator simulates lightning via an electrical discharge field.  The immune system also naturally forms ozone in our bodies to kill invading bacteria, viruses, parasites, and fungi. It works because oxygen is the only gas with a terminal electron receptor and can carry an electrical charge.

Contrary to common misconceptions, ozone is not a pollutant but a highly reactive form of oxygen (O3) that is non-toxic to human cells at the concentrations used in dental applications. Ozone effectively targets and destroys disease-causing microorganisms such as bacteria, viruses, and yeast by attaching to their positively charged surfaces, as it carries an electronegative charge. When introduced into the body, ozone triggers a series of reactions that produce hydrogen peroxide and other compounds, creating an oxidative burst that microorganisms cannot withstand. Additionally, ozone boosts the immune response, stimulates blood flow, and enhances healing, reducing infection risk post-surgery. In dental practices, ozone is used in various forms—such as ozonated water, oil, and gas—for treatments that include cleaning and disinfecting teeth and gums.

Here are some uses of ozone

Periodontal Issues: When it comes to periodontal issues, our practice employs ozone therapy. This multi-step process begins with irrigating the area with ozonated water and directly infusing ozone gas into the gum pockets. This method has shown positive results, with reduced pathogens, increased blood flow, and improved healing. Patients can also continue this treatment at home with ozonated olive oil, ensuring a comprehensive and effective approach to gum disease.

Surgical Procedures and Implants: Ozone is part of our implant and extraction protocol. When we remove a tooth, we do an atraumatic extraction. To do this, we use Piezosurgery, a procedure involving ultrasound waves cutting into bone. We use gentle pressure at key points to use leverage instead of brute force. The waves do not damage surrounding tissue, giving Piezosurgery its distinct advantage. We will then remove the periodontal ligament, disinfect the socket with ozone and laser, and place a PRF membrane obtained from the patient’s blood draw. We have had great results with this protocol. The healing is excellent, and the pain is minimal. Maintaining the bone between the roots (interarticular bone) allows us, in most cases, to place a ceramic implant immediately after extraction.

Cavities: We use ozone on cavitiy and crown preparations. Ozone disinfection properties help prevent small cavities and inhibit deeper cavities from degenerating into root canals. It is especially useful for treating cavities in children, as minimal drilling is all that is required. I have treated small children with ozone trays who were recommended to be put under general anesthesia. We gained more time, allowing the child to mature enough to handle single fillings.
